Dextrocardia

cakap-cakap shahied

Had a patient today during the tutorials.

18 year old girl

Came in with sore throat, fever, SOB, productive cough.

Has a background history of recurrent chest infections. Almost once every month.

So we asked a few questions.

At first thinking that this cud be cystic fibrosis, or bronchiectasis, or some sort of chronic lung disease, or immunosuppression.

The we were told to examine the cardivascular exam.

So we did.

Started at the Aortic area, fine, Pulmonary area, fine, Tricuspid area fine, the Mitral area.......silence..i cudn't hear anything. HMM..maybe i got the wrong spot.so i moved up a little bit.Still nothing. Being that it was a young girl, i was quite hesitant to explore further, assuming it was just me that cudn't hear it. But then my 2 collegues said they could hear it at the apex beat. I was more puzzled.

The we auscultated the basal lung areas. Definitely reduced air entry at the RIGHT side.Infact, it was almost silent. So we thought, fine, probably theres a pleural effusion there.

So after finishing we went out to discuss the findings with the Registrar. So she confirmed my findings, much to the bemusement of my two collegues that there was definitely no sound at the apex beat.

"So where is it then?" she asked. Lets look at the x-ray she said.

Lo n behold, there it was, the heart, on the RIGHT side of the lung. I smiled at Mr. S who confidently said he heard a heart sound at the apex beat which was on the left.

Apparently, this was a case of probably an apparent dextrocardia, not a true one.

Possible Kartagener's Syndrome (situs inversus, chronic sinusitis, bronchiectasis)

you do not want to be labelled as a "L"!!

cakap-cakap maestronine

you know that you don't know, yet you do nothing about it..

you know you have many things to do, yet you are still relaxing..

you know you are facing a tough and strong opponent, yet you do not train..

you know you have prioritise your agendas, yet you still do things which are least important..

you know you have a write-up to be submitted, yet you keep procrastinating..

you know exam is around the corner, yet you are still messing around..

those are just random examples.. seriously you do not want to be called a loser right?? the good thing is you still have the time to change things the way you want it to be.. so start changing.. now or never.. lets be a winner.. berusaha!!

Why I miss my DAD...

cakap-cakap izadiskandar

"Ada 1 advert. Petronas CNY 2009, pasal kisah seorang papa & mama. Shd Watch at utube if free"

That is the text/SMS I got this morning from MAMA when I was doing Ward rounds. I wondered what this video was. So after Outpatients Clinic session ends around 12, i went home, stopped over at Farash store, get my chickens, cook lunch/dinner for the boys and log on to check youtube.

Watching it, I suddenly miss my papa, and my mama... and wondered how they are..

So yes, I miss my Papa, and Mama too.
Do u notice any similarity between this picture and the advertisement above?

HAPPY CHINESE NEW YEAR FRIENDS.

ketam melayu ... ketam cina

cakap-cakap ilman aiman

just now, we had this discussion where it led to this story. and for those who are unaware of the metaphors in between the words, then this is a story about the Malay, the Chinese, & success...

once upon a time in a land far far away..

Melayu : Apa pasal ketam cina apek kasi tutup…ketam melayu apek tak kasi tutup?

Macam ketam melayu, bila sekor ketam dah berjaya nak naik, ketam lain tarik kaki dia, at last sorang pun tak dapat naik . Lepas tu mulala bergaduh sesama sendiri. Rata² melayu orang melayu macam tu. PHD (Perasaan hasad dengki) begitu tebal dalam diri mereka.

p/s: this has nothing to do with anybody. just food for thought

abuse..

cakap-cakap maestronine

i would agree with prof.. abuse can be in many forms.. and in the context of paediatrics, ~99.99% of children have been "abused" by their parents at least once, during his/her life.. for instance, according to him, parental smoking can be considered as one form of child-abuse.. it never came to my mind but i think it's true to a certain extent.. abuse is absolutely a thought-provoking topic to be discussed and talked about.. different people may have different definition and threshold with regard to what is and what is not abuse.. as well as the gap between "abuse" and trying to instill some discipline and positive attitudes into their children.. so tepuk dada tanya selera.. from my point of view, we just need to have the right "nawaitu" and rationalise our actions and behaviour, keeping in mind the things we do are for the good of ourselves and people around us..

on a separate issue, yeayy 1st time this morning on the way to cuh.. saw a 09-C-xxxx red car.. finally.. i don't know why i am making such a fuss about this.. but i think it's just me.. haha =p

alright lads.. have a good night sleep ok.. dream of me *wink*
take care..

Perghhh.. pehal jiwang sangat ni??

cakap-cakap shahied

Love. Its a weird thing. Its natural to want to be loved, to love. Sunnatullah we are made as men and women in pairs. Everybody has their own definitions of what love is. "How do you know if you love someone?" a friend once asked me. My answer was, well, until you yourself experienced it, its hard to explain. You would know it yourself if you are (i think). This is not a general rule though. Of course there are exceptions to everything.

Love. Its spans all humanity.

Question is, if you were given a choice, and you MUST ONLY CHOOSE ONE, which one would you prefer?
a) being with someone who loves you
or
b) being with someone you love

my theory was, guys would mostly choose b), and girls would mostly would choose a)
anybody dare to prove me wrong??
